The following registration forms are for our upcoming events only and will need to be printed, filled out and mailed in along with payment. We apologize, online registration for these events is not available at this time. Foundation donations can be made separately here.

LEMF Charity Dinner and Auction
Don't miss the First Annual Lauren Elise Memorial Foundation Charity Dinner and Auction! The inaugural event will be held on Friday, June 1st, 2012, at beautiful Five Oaks Lodge in Jenks (www.fiveoakslodge.com). Doors open at 6:00 PM. This will be a wonderful night at a beautiful venue, with dinner, and great auction items available for bid! All proceeds will go to support LEMF and it's mission! Only a limited number of tickets are available, so please reserve yours today. Ticket prices are $85 per person, or $160 per couple. 1st Annual LEMF Charity Golf Classic
Please plan to join us for the first annual Lauren Elise Memorial Foundation Charity Golf Classic! The event will be held at White Hawk Golf Club in Bixby, OK (www.whitehawk.americangolf.com), on Saturday afternoon, June 2nd, 2012. The event will kick off with lunch at 12:00, with a 1:30 shotgun start for the tournament. Tournament format will be a 4-man scramble, with prizes for winners, as well as for individual challenges such as long-drive and closest to the pin competition! Whether you're a scratch or a hack, this will be a fun event, with all proceeds going to support the foundation! Entry fee is just $75 per player, or $280 for a four man team. Fee includes lunch, player gift bag, and tournament entry. Complete the attached LEMF Golf Classic entry form, and send along with your payment to PO Box 101, Jenks, OK, 74037-0101, to reserve your spot today! Only a limited number of spots are available.

Joint Event Attendance
For those who have interest in supporting both events, first let us say thank you! Those who purchase tickets for both events by registering up front for joint event attendance, will receive discounted pricing. Platinum Level - $500
Includes hole signage, exclusive sponsorship of a hole at the LEMF Charity Golf Classic. In addition Platinum sponsors will receive exclusive sponsorship of a table at the LEMF Charity Dinner and Auction, and will receive special thanks and recognition on our website!
Gold Level - $250
Includes hole signage at the LEMF Charity Golf Classic, indicating your company is a Gold Sponsor at a particular hole. Max two Gold Sponsors per hole.
